I had to work on a camper back in the '80s.  It had clutch head screws and I had to buy a whole assortment of drivers to get one.  Now, they seem to moved to square drive (of which I have lots).

 

We used to get a lot of cam-out on screws on the theater set construction.  People would just go crazy.  I convinced the manager to go to Torx and most of that has stopped, except for one guy last weekend that both messed up the screw and messed up the driver.  As soon as you make something fool-proof, a bigger fool comes along.

 

Probably the worst (besides the slotted, IMO) is the combo drives.  None of them seem to work well.  I've had a few customers mess up the square+phillips and I pull out my square driver and get it out and they look in amazement.

my favorite story was from many years ago, a crew was assembling some steel studs for an interior space.  and using little green screws to hold the parts together.  one big guy, worked all day, only to discover, at the end of the day, that his screw gun had been set to "counter clockwise" all day.  hey, the screws went in, didn't they.
