Ron, it's done freehand. Most guys who are piercing at that level are using an NSK Presto dental hand piece. Or, at least it's based on one of those. They are pneumatic and turn at about 350,000 rpm and it's important that the work is around 1/16" thick. It's normal to use carbide or diamond burs less than a mm in diameter. I have done something similar  but use a much cheaper version I got on Ebay.

Like Steve said if you can turn thin , that is 1/16 the piercing is easy . The pro tools run upwards of 300 to 400. Captain Eddie has a video showing a cheaper way to get there and it is also a NSK. You can find then on ebay under 20. Then just a few more parts and HF has that.
