My initial guess is that this is a bender to birng two wood strips into alignment and hold it there.  I base this on two things.  First is that there is the bottom mortis that could hold a strip.  Then there is the top mortis that will rotate.  Since there is a pin in the middle, I would assume that this would apply to the ends of the woods.

It's all handmade, top rotates to the side so something can be put into it. the bottom has a groove seperated by a bolt which suggests that part ran on 2 flat bars back and forth.



The top also has gear teeth that can be adjusted right side/left side as needed suggesting different angles.



My best guess is a part off of some grinding/sharpening jig.

Maybe something to do with those leather drive belts that powered the old mill machines.     Use it to dress the ends of a belt, before joining the neds together.    One end held below is the one to be matched.    Top slot is to hold the other end,  cog lever  can set it to an angle for a Furring Plane to dress to end. 




IF a belt breaks, ends might a bit jagged, and torn up.   This would be a way to match the ends up, so the belt will run straight, and true.

I'm going with the Medieval Dentist. I think it clamps to your tongue so they could pull it out of the way.
